Immunization requirements and recommendations

RISD Health Services is dedicated to providing a safe and healthy environment on campus. As part of this commitment and in compliance with Rhode Island State Law, we require health forms and immunization records from all students, including commuters and graduate students.

Due to the intensive studio-driven nature of RISD programs and the global composition of our community, we firmly believe that public health data for our campus is vitally important! We are fortunate to have an active and tightly knit community here at RISD. Even students who live off-campus still spend a significant amount of time on campus and in studios. In the event of an infectious disease outbreak, Health Services needs accurate data to be able to isolate appropriately and get quick care for those who are at risk.

Please be sure that you have submitted your required health forms and immunization records to RISD Health Services. For those who have not yet submitted health history forms and vaccine records, please do so through our online Patient Portal.

Which vaccinations are required by the State of Rhode Island?

  • COVID-19 series + booster
  • 1 Tdap
  • 2 MMRs
  • 3 Hepatitis B
  • 2 Varicella
  • Meningitis (21 years and under)

Students may request a medical or religious exemption to RISD’s student vaccination requirement. To do so, contact Health Services at health@risd.edu to request an exemption form, and complete and submit the exemption form for review.
